This website was created by Rasheeda Stuart, Judith Perryman & Shawnico King who are all second year General Nursing Students of the Barbados Community College. We were tasked with constructing a website based on the Assessment of the Respiratory System. Our aim was to not only complete the assignment we were given but to also educate future nurses, our fellow colleagues & also any one curious about what goes into a focused respiratory assessment. We hope that this website will be helpful in whatever endeavor you came to pursue.

Scenario
Situation: A group of students are assigned to a medical unit and the time is now 1400 hours. The students are caring for Ms. ACT, a 41-year-old female who was admitted yesterday.
Background: The patient has had fever and chest pain over the past 2 days.
Assessment: Her vital signs were assessed 4 hours ago. Temperature has been steady, around 39 oC, SpO2 was 98%, RR 20/min, BP 143/92 mmHg and HR 83/min.
Recommendation: She is due for a reassessment and the Registered Nurse has asked the students how they would conduct a focused respiratory assessment on Ms. ACT.

What is Health Assessment?
A health assessment is a plan of care which focuses on the specific needs of a person and ways these needs would be addressed by the healthcare team. It also evaluates a person's health status by taking a health history as well as performing a physical examination which includes: Inspection, Auscultation, Palpation and Percussion.

What is The Importance of a Health Assessment in Nursing?
It is a critical component of every care plan, track status change and goal progression. To conduct accurate assessments, nurses must have strong attention to detail and a deep understanding of body systems. Nurses should continue to expand and refine their assessment skills throughout their careers.

What is a Focused Assessment?
Focused assessments are performed by healthcare professionals in response to a specific patient health problem recognized by the assessor as needing further assessment of a body system or systems.
Focused Assessment of the Respiratory System
A focused respiratory system assessment includes the collection of subjective data about the patient’s history of smoking, collecting the patient’s and patient’s family’s history of pulmonary disease. Asking the patient about any signs and symptoms of pulmonary disease, such as cough and shortness of breath. Objective data is also assessed.
